 Financial Aid Application Checklist

If you plan to use federal student loans in payment of your educational expenses, you must complete the following steps:

1. Get Your FAFSA PIN
If you have never completed a Free Application for Federal Student Aid (FAFSA), we recommend that you request a Personal Identification Number (PIN)

The PIN serves as an electronic signature on the FAFSA/Renewal FAFSA, as well as allowing access to other federal student loan data systems. It should be kept in a safe place for use in future years.

2. Complete the FAFSA – School Code: G20961
You must complete the Free Application for Federal Student Aid (FAFSA) form every academic year to assess your eligibility for federal student loans.

If you are a Fall 2013, Spring 2014, or Summer 2014 term applicant or renewing student, use the 2013/2014 FAFSA (with your submitted 2012 tax documents).

If you are a Fall 2014, Spring 2015, or Summer 2015 term applicant or renewing student, use the 2014/2015 FAFSA (with your submitted 2013 tax documents).

3. Complete the Loan Request Form
Download the Federal Direct Student Loan Request and Acceptance of Awards form matching the FAFSA you submitted:

2013/2014 FAFSA (2012 tax year) - 2013/2014 Loan Request and Acceptance of Award form

2014/2015 FAFSA (2013 tax year) - 2014/2015 Loan Request and Acceptance of Award form

Complete the required fields, then print, sign, and fax to 805.690.4339, or e-mail a signed, scanned copy to finaid@fielding.edu

You must indicate the dollar amount you wish to borrow based on your personal situation and the tuition and other estimated expenses that make up the cost of attendance budget.

4. Complete Federal Student Loan Entrance Counseling
Entrance counseling is required once for new loan applicants at Fielding Graduate University. Be sure to indicate that you are a Graduate Student.  Automatic notification of your successful completion will be sent to Fielding directly.

5. Complete a Master Promissory Note
The Department of Education requires that you submit a Master Promissory Note (MPN) for your Stafford Loan.

If you plan to borrow more than your annual maximum in Stafford loans through a Grad PLUS loan, you must also:

6. Check your E-mail – Additional Information may be Required
Check your e-mail regularly for messages from the Financial Aid Office. You may be selected for Verification, or your FAFSA may indicate the need for additional documentation. You will receive an e-mail indicating any additional forms or information that may be needed. Please respond promptly in order to expedite your loan processing.

Receipt of all information must be received by the Financial Aid Office according to your program admission calendar or your renewal deadline to ensure timely processing. Late receipt or incomplete materials cannot guarantee processing will be complete prior to your tuition due date.
